We offer a UNIQUE Voluntary mental health/Medical treatment plan which utilizes the collaboration of all disciplines (RN, LPN, PT, OT, HHA) to individualize each care plan.
Our goal is to promote independent living in the community and prevent re-hospitalizations through Care Plan Development, Medication Management, Crisis Intervention, Coordination of Care, Community Resources Coordination, Communication with Providers and Disciplines.
The Behavioral Health Services include the following:
- Assessment and individual treatment planning
- Plan of care based on reality of client’s life and home situation. It addresses health status, knowledge and self care skills
- Medication monitoring, including administration of deconoate medications to include: Haldol & Prolixin, as well as Invega Sustena, Vivitrol, Ability Maintena, and Risperdal Consta injections
- Behavioral Management
- Crisis Intervention
- Telephone Support
- Psychiatric nurses manage the care of the client through interdisciplinary teamwork:
- The nurse may make the initial contact with the client in the hospital
- The nurse participates in the discharge planning meetings
- The nurse collaborates with all disciplines involved by communication what is happening in the care of the client through regular telephone contact to the MD, case managers and other mental health professionals involved in the care of the client.

Plays an important role in maintaining care for patients at home, concerning the more complex tasks regarding medical health that needs to be addressed accordingly. Among the services they provide are the following:

•Psychiatric Nursing
Psychiatric nurses are specialty nurses trained in the field of caring for patients who have mental challenges, disorder, and distress such as those with Bipolar Disorder, Depression, and Dementia. They provide psychological therapy services for patients regardless of age with the aim to guide, treat, and maintain their psychological and mental well- being.
They are engaged in talking and counseling sessions with patients, aside from providing basic nursing care services and meet ANCC Certification Guidelines.
•Case Management
Case managers are licensed social workers who coordinate services in behalf of the patients and their families. Case managers serve as community liaison officers who make community resources accessible to the families and their needy loved ones.
They conduct assessments, set goals with the family members, and develop long term care plan for the patients’ advantage and benefit, concerning health. They work with the family for a longer span of time, occasionally meeting and constantly communicating with them to update the development of the work in progress.
